A simple example of a Risk Register in a hospital is shown in Figure 1.1, where certain types of risk events (e.g., wrong dosage given, equipment failure, etc.) that have occurred within specific departments (e.g., surgery, intensive care) and the number of events that happened within a specific time period are recorded, as well as other qualitative notes and associated details. Reports are then typically generated. Figure 1.2 shows a sample periodic (e.g., monthly) report of another organization showing the number of risk events that occurred in the past.
